Add Questionnaire Button to Service Request

Is there a way to add one or more questionnaires to buttons in a Service Request?  We've got varying scenarios within a Request that are not appropriate to add to the workflow since they may or may not be applicable to each request and only needed on demand.

Hi,
I have exactly the same question.
I'm actually looking for a way to do that...
I'm exploring a clue using a Flag field and business rule.
If the case is check a rule lunch the questionnary... But I'm not sure yet if this is possible. I'm awaiting for an anwser from my support team.
