Extra question on multi-section questionnaire when viewed from Service Apps

We have a multi-section questionnaire that is to be completed as an action of a service request workflow. I have set up a service app to complete these specific actions so the questionnaire is opened up in the app (more responsive/autofill/etc). However on the very last section of the questionnaire a random question shows up at the very end. This only occurs when the questionnaire is opened from a service app, if it is opened from the service manager it shows up normally. I have attached screenshots of how the sections look in both SA and SM.

A few more details:
- the question populates no matter which section is at the end, it always shows up on the last section at the bottom
- the question shows up at all multi-section questionnaires, not just the one in the screenshot


Has anyone experienced anything similar? I have been combing through all of the questions and settings trying to find how this is being input on the end of the questionnaire but have had no look.

Hello,

"End date; Duration and Comment" are add by default to questionnaire (in Apps) when type of action are "Operation Action". Try to change to "Validation selfService" for not display this fields.

Hello,
